We are a not-for-profit organisation based in Tasmania, we deliver services across four key pillars: Primary Health Services, Disability Services, Aged Care Services, and Community Innovation. We recruit and support health professionals, provide NDIS and aged care support, and develop programs that strengthen communities.

We’re a Tasmanian not-for-profit organisation dedicated to helping people access the care, services, and opportunities they need to live well.

With offices in Launceston, George Town, and Devonport, we combine local knowledge, expertise, and a deep understanding of our communities to deliver practical, personalised support across Primary Health, Disability Services, Aged Care, and Community Innovation.

Our approach is grounded in connection, compassion, and collaboration. We work alongside individuals, families, service providers, and communities to ensure every Tasmanian can access the right support at the right time. With a focus on integrity, flexibility, and genuine care, we help people achieve their goals, strengthen communities, and build healthier, more inclusive lives.
