Cygnet Family Practice are seeking an enthusiastic, Nurse Practitioner to join their urgent and after-hours service. The Nurse Practitioner will be able to work autonomously and collaboratively, making high level decisions to treat health needs in a practice setting, and in the home environment. This service is part of an exciting development to deliver an integrated hospital admission avoidance service in conjunction with local general practices, pharmacies, Tasmanian Ambulance Service, Primary Health Tasmania, and the Tasmanian Health Service. This role is a ground-breaking opportunity and will demand excellence and a tireless commitment to the success of the role to forge a legacy for nurse practitioners nationally in autonomous and collaborative practice.

The Position

Key responsibilities include:

  • Advanced clinical assessment and diagnosis
  • Prescribing medications and ordering diagnostics
  • Coordinating patient care with multidisciplinary teams
  • Responding to emergency callouts as required
  • Applying evidence-based research to practice

Essential criteria

  • AHPRA registered Nurse Practitioner with significant clinical experience in urgent, emergency, and primary health care
  • Advanced/extended clinical knowledge pertaining to primary health issues and management of deteriorating patients
  • Understanding of clinical research and its application into practice
  • Ability and willingness to train and mentor registered nurses and community paramedics
  • Evidence of continuing professional development
  • Aptitude for IT based applications utilised in Primary care
  • Ability to initiate sustain, drive. and evaluate change
  • Member of the Australian College Nurse Practitioners

This role is based in Cygnet, a welcoming township in the stunning Huon Valley, just 50 minutes south of Hobart. The Cygnet Urgent and After-Hours Service has been operating for over 2.5 years and is funded by the Tasmanian Government to improve healthcare access for vulnerable cohorts. Using a unique model combining Nurse Practitioners and Community Paramedics, the service is a key part of an integrated health initiative aimed at reducing unnecessary hospital admissions and improving continuity of care in rural Tasmania.
